Erectile Dysfunction & SMC-K: VA Claims & Ratings

ED is rated at 0%, but that's not the end of the story. Loss of use can qualify for Special Monthly Compensation (SMC-K), a monthly payment on top of your regular compensation.

The rating: DC 7522

Under 38 CFR § 4.115b, DC 7522, erectile dysfunction (with or without penile deformity) is rated at 0%. The schedule directs VA to consider special monthly compensation whenever ED is service-connected.

SMC-K: the real benefit

SMC-K is paid for loss, or loss of use, of a creative organ (38 CFR § 3.350). Effective December 1, 2025, SMC-K is $139.87 per month, added to your regular compensation at any rating from 0% to 100% (VA SMC rates). A veteran can receive up to three SMC-K awards for different qualifying losses.

Paths to service connection

  • Secondary (most common): ED caused by service-connected diabetes, PTSD or depression (and their medications), spinal conditions, prostate cancer treatment, or hypertension medications
  • Direct: an in-service injury or condition

Your treatment records should document the ED, its likely cause, and whether it persists despite treatment. A focused medical opinion helps when the link isn't obvious.

It's a medical claim, and it's confidentialMany veterans never bring this up. That's understandable, but it's a legitimate, common claim, and we handle it with professionalism and discretion.

Questions

Frequently asked questions

Does a 0% rating pay anything?

A 0% rating alone doesn't pay. But SMC-K is paid even with a 0% rating, and the 0% rating still counts as service connection.

Does SMC-K affect my combined rating?

No. SMC-K is a flat monthly amount added to your compensation. It doesn't change your combined percentage.
